This is a tricky one because technically you cannot put NO SUBSTITUTION on generic or non-proprietary drug names.  If the doctor had written Apo-Tetracycline you would have to dispense the Apo brand of tetracycline.  I would call the MD and ask what he meant by NO SUBSTITUTION on tetracycline since the intent of the MD is unclear.
